The First re-registration Students who can successfully satisfy the Academic Potential Test (TPA) will receive confirmation of admission via SMS. The confirmation of admission can also be accessed through:

Few days after the confirmation of admission, students are required to take the re-registration documents (within one folder) at their respective High School or at Ubaya (at the admission and registration counter).
The re-registration documents consist of:
The USP Confirmation letter
The bio data scanner sheet
The Student’s interest, aptitude and ability sheet
The Bank Mandiri Account Opening form
Re-registration documents must be filled out and must be returned to students’ respective high school or to Ubaya, along with a copy of the receipt of 1st USP instalment payment. Therefore, the re-registration documents that must be submitted are as follows:
a copy of the receipt of the 1st USP instalment payment or USP settlement payment
The Student’s interest, aptitude and ability sheet (along with 2 pieces of 3x4 photo)
The drug-free statement letter and the resignation rules (with stamped paper)-forms are available on appendix 4
a piece of 3x3 photo (for making a student candidate ID card)
The Bank Mandiri Account Opening form and a copy of Citizenship card/Driving license (could be submitted on the second re-registration)
Few days after the high school (SMA) sends the re-registration documents to Ubaya, Ubaya will issue a student candidate ID card (KCM) as the official proof that the relevant students have been registered as Ubaya student candidate. All cards will be sent directly to students’ respective high schools. Students are required to take their KCM at their respective s high school or at Ubaya (for students who re-register in Ubaya).
The final step will be the second re-registration.
The Second Re-Registration Prior to the commencement of study period, new students are obligated to carry on the second re-registration that consists of opening Bank Mandiri account, completing the administration and obtaining students’ rights as Ubaya student candidate. The aforementioned steps should be done at the re-registration venue at Ubaya Tenggilis campus.
Steps that must be done for this stage are as follows:
Obtain the invoice available at Ubaya cashier
Settle the payment determined on the invoice
Go to Bank Mandiri counter at Ubaya (if there is unsettled payment)
Submit the complete documents that consist of:
2 copies of a birth certificate
2 copies of the high school certificate which has been legalized (STTB/STK) (could be submitted afterwards)
a copy of the National Examination (Unas) transcript (could be submitted afterwards)
For Pharmacy, Chemical Engineering, Biotechnology, and Product Design & Management, student is required to provide the statement letter from doctor states that student is not colour-blind and physically capable of conducting laboratory works
a copy of citizenship card (for those who have not submitted the card on the first re-registration)
- Photo taking for the issuance of Student ID Card (KTM). KTM is a magnetic card and is a replacement of Student Candidate ID card (KCM)
- Obtain University Jacket and Muts
- Open Bank Mandiri account with the initial balance of Rp. 10.000,00. Note: Please make sure that the signature is the same with the one in the citizenship card.
Cooperation-based Track (JKS) and Normal Track

Afterwards, students are required to take the re-registration documents (in 1 document) at the same venue where students submit the registration forms. Note: Re-registration documents can only be obtained once the students submit the TPA card.
The re-registration documents obtained by students will consist of:

Those who conduct their first re-registration at Ubaya could possibly conduct the second re-registration the same day with the first re-registration. While, for students who conduct the registration process at high school (SMA) could conduct the second re-registration prior to the commencement of lecture period.
